A social services navigation program for Medi-Cal enrolled members
About nha cARE lINK
Care Link program is a Medi-Cal managed care benefit that aims to address the holistic needs of enrolled members. The program provides both Enhanced Care Management (ECM) and Housing Navigation. Through these services, members can receive well-rounded care to address health and health-related needs. These two tracks work collaboratively depending on the individual’s needs to help members create effective goals.
Services
Enhanced Care Management (ECM) Services:
- Care Coordination Services
- Health Promotion/Education
- Transitional Care Support
- Referral Coordination to Social Services
- Services are offered in English and Spanish
Housing Navigator Services:
- Housing Transition Navigation Services
- Housing Security Deposits and move in costs
- Housing Tenancy & Sustaining Services
- Short Term Housing Assistance
- Develop a spending plan and budgeting system
- Provide support to individuals and families with housing applications
- Coordinate with ECM to assure clients continue with support services upon housing placement.
- Provide local housing resources, including emergency shelters, transitional housing, and permanent housing.
- Provide clients with information on tenants’ rights, responsibilities, and resources to support their housing tenancy.
Eligibility
- Enrolled members of Medi-Cal over 18 years of age
Payment
Care Link services are offered through Medi-Cal at no cost to members.
