About Adult Day hEALTH Care (ADHC) AND Adult Day ProgRAM (ADP)

Adult Day Health Care provides two program treatment options for adults 18 years of age and older. Adult Day Health Care (ADHC) and Adult Day Program (ADP) are person-centered care environments. ADHC is designed for adults with disabling physical, mental, cognitive and/or intellectual impairments providing participants with nursing, rehabilitative (OT, PT and ST) therapies and psycho-social services. ADP focuses on memory care and personal care assistance, while providing a variety of cognitively stimulating activities in a “home away from home” environment. ADHC and ADP provide transportation services, meals at lunch and snack, and fun recreation activities are available to all participants at the center.

*ADP program provides limited nursing services

Eligibility

Adult Day Health Care is open to adults 18 years of age and older, whose needs include: 

  • Ongoing nursing care and skilled observations (such as diabetic care management)
  • A medical condition that requires treatment (such as rehabilitative therapies) ordered by a physician
  • Chronic mental health, intellectual disabilities, or cognitive impairments associated with a diagnosis of dementia that effects the ability to perform certain daily living activities

Payment

Adult Day Health Care (ADHC) is a fee-for-service program. ADHC services will be offered through Medi-Cal/CBAS, VA Contract, Regional Center Contract, and private pay options. Adult Day Program (ADP) services are private-pay only.
